Effective Date: December 15, 2016
Last Updated: May 31, 2026
CG Cosmetic Surgery ("CG Cosmetic," "we," "us," or "our") is committed to protecting the personal information you provide to us. This Privacy Policy explains what information we collect through this website (cgcosmetic.com), how we use and share it, and the choices and rights available to you.
Please read this Privacy Policy carefully before using this site. This Policy applies to information collected through this website. It does not cover information collected offline, through separate sites operated by our affiliates or business partners, or through your treatment relationship as a patient, which is governed separately by our HIPAA Notice of Privacy Practices.
This site is not directed to and is not intended for individuals under 18 years of age. We do not knowingly collect personal information from anyone under 18.
1. Information We Collect
Information you provide to us. When you submit a form, request a consultation, communicate with us, or otherwise interact with the site, we may collect your name, email address, phone number, mailing address, procedures of interest, and any information you choose to include in a message.
Information collected automatically. When you visit the site, our servers and analytics tools automatically collect certain technical information, including your IP address, browser type and version, device and operating system information, the referring website or ad, pages viewed, links clicked, and the dates and times of your visits. We use cookies, web beacons, pixels, and similar technologies to collect this information (see Section 4).
Location information. If you use location-enabled features, you may send us approximate location data. These features are opt-in and can be turned off at any time through your device settings.
2. Health Information and HIPAA
We treat your health and medical information with care. We do not share any medical, health-condition, treatment, or diagnosis information with advertising platforms or analytics providers. Information you provide as part of a treatment or patient relationship is protected health information governed by HIPAA and our Notice of Privacy Practices, not by this website Privacy Policy.
Please do not send highly sensitive information - such as Social Security numbers, credit card numbers, or detailed medical histories - through unsecured channels like standard email or web forms.
3. How We Use Your Information
We use the information we collect to:
- Respond to your inquiries and consultation requests;
- Provide, operate, and improve the website and our services;
- Measure and analyze site traffic, performance, and usage;
- Deliver, measure, and optimize advertising (see Section 5);
- Send promotional and marketing communications where you have consented;
- Detect, prevent, and address fraud, security issues, and technical problems; and
- Comply with legal obligations and enforce our terms.
4. Cookies and Tracking Technologies
We use cookies and similar technologies for essential site functions, analytics, and advertising. On your first visit you are presented with a consent banner allowing you to Accept All, Reject All, or Manage Preferences across the following categories:
- Essential - required for the site to function; always on.
- Analytics - helps us understand how visitors use the site (e.g., Google Analytics). Data is aggregated and de-identified.
- Advertising - shares limited demographic and behavioral data with advertising partners to deliver relevant ads. No health or medical data is shared.
You can change your cookie choices at any time through the "My Privacy Choices" link on the site, or by configuring your browser to reject or alert you to cookies.
5. Advertising and Third-Party Platforms
We advertise and measure campaign performance across several platforms. To do this, we share limited, non-medical information - such as a hashed email or phone number, device identifiers, and browsing/conversion events - with the following partners. We never share health conditions, procedures booked, or diagnosis information for advertising purposes.
Meta (Facebook and Instagram). We use the Meta Pixel and Conversions API to measure ad performance and reach relevant audiences on Facebook and Instagram. Information shared may include hashed contact identifiers and on-site activity. See Meta's Privacy Policy.
Google (Ads, Analytics, Tag Manager). We use Google Ads, Google Analytics, and related tagging to measure traffic and ad performance and to deliver relevant ads across Google's network. See Google's Privacy Policy. You may opt out of personalized Google ads at Google Ad Settings.
OpenAI. We use OpenAI services to power AI-assisted features such as automated chat, lead engagement, and content tools. Information you submit through these features may be processed by OpenAI to generate responses. We do not permit this information to be used to train third-party models. See OpenAI's Privacy Policy.
These partners may set their own cookies and process data under their own privacy policies. The information shared contains no medical or health-condition data.
Our advertising conversion events are configured to be general (for example, "lead" or "form submission"). We do not attach the specific procedure, treatment, or page of interest to any advertising or conversion event, and we do not build advertising audiences based on the specific procedure a visitor viewed.
6. How We Share Information
We may share your information with:
- Service providers who help us operate the site and our business, including our customer relationship management (CRM) and messaging platform (GoHighLevel), website hosting, analytics, and payment processors, bound by confidentiality and data-protection obligations;
- Advertising and analytics partners as described in Section 5;
- Legal and safety recipients - law enforcement or government authorities when required by law or to protect our rights, property, or safety; and
- Business transfers - a successor entity in connection with a merger, acquisition, or sale of assets.
We do not sell your personal information for money. Certain advertising-related data sharing may be considered a "sale" or "sharing" of personal information under California law; you may opt out as described in Section 7.
7. Your Privacy Rights
Depending on where you live, you may have the right to:
- Access or obtain a copy of the personal information we hold about you;
- Correct inaccurate personal information;
- Request deletion of your personal information;
- Opt out of targeted advertising and the "sale" or "sharing" of your personal information; and
- Not be discriminated against for exercising these rights.
California residents (CCPA/CPRA) and Florida residents (Florida Digital Bill of Rights), as well as residents of other states with applicable privacy laws, may exercise these rights. To opt out of advertising-related sharing, use the "My Privacy Choices" link on the site or adjust your cookie preferences. To exercise any other right, contact us at request@cgcosmetic.com. We will verify your request and respond within the timeframes required by applicable law. You may use an authorized agent to submit a request on your behalf.
We recognize the Global Privacy Control (GPC) browser signal. If you visit the site with GPC enabled, we treat it as a valid request to opt out of the "sale" or "sharing" of your personal information for that browser.
If we deny your request, you may appeal by replying to our decision or contacting us at request@cgcosmetic.com with "Privacy Appeal" in the subject line. We will respond to your appeal within the period required by applicable law.
8. SMS / Text Message Communications
When you opt in to communicate with us by SMS/text, we collect your phone number, the content, date, and time of your messages, and any information you provide.
We use this information to respond to your inquiries, provide requested services, and - with your consent - send appointment and promotional messages. SMS consent and your phone number are never sold, shared, or disclosed to third parties for marketing purposes. We may share message data only with service providers (such as our messaging platform) that help us deliver the service, and with authorities where required by law.
Message and data rates may apply. Message frequency varies. Reply STOP to opt out at any time, or HELP for assistance. Opting out of SMS does not affect your other interactions with us. For full details, see our SMS Terms and Conditions.
9. Data Retention
We retain your personal information only for as long as necessary to fulfill the purposes described in this Policy, including to respond to your inquiries, provide our services, comply with our legal obligations, resolve disputes, and enforce our agreements. When information is no longer needed, we take reasonable steps to delete or de-identify it.
10. Data Security
We maintain reasonable physical, electronic, and procedural safeguards designed to protect your information against loss, misuse, and unauthorized access, disclosure, alteration, or destruction. Access to personal information is limited to employees and agents who need it to provide our services, and they are bound by confidentiality obligations. No transmission over the internet or wireless network can be guaranteed to be completely secure.
11. Children's Privacy
This site is not intended for individuals under 18 years of age, and we do not knowingly solicit or collect personal information from anyone under 18.
12. United States Jurisdiction
This is a United States-based website operated by CG Cosmetic Surgery from its offices in the United States and subject to U.S. law. If you access the site from outside the United States, you do so on your own initiative and agree that your use is governed by this Privacy Policy and U.S. law.
13. Changes to This Privacy Policy
